超声右心功能指标与慢性阻塞性肺疾病患者平均住院日和住院费用的相关性
Effect of Pulmonary Hypertension on Average Length of Stay and Cost of Hospitalization in Patients with Chronic Obstructive Pulmonary Disease

Abstract
Objective To investigate the correlation between ultrasound right heart function index and average length of stay and hospitalization cost in patients with chronic obstructive pulmonary disease(COPD).Methods From January 2020 to August 2020,104 patients with COPD were enrolled in the Department of Respiratory Health,Guangzhou Institute of Respiratory Health,the first affiliated Hospital of Guangzhou Medical University.There were 59 cases in COPD group and 45 cases in COPD with PH.The general data,pre-brain natriuretic peptide(npro-BNP),right atrium diameter(RA diameter),right ventricular transverse diameter(RV diameter),stroke volume(SV),ejection fraction(EF),pulmonary artery systolic pressure(PASP),average hospitalization days,hospitalization expenses and western medicine expenses were compared between the two groups.Results There was no significant difference in age,BMI and EF between the two groups(P＞0.05).There were significant differences in NT-proBNP,RA inner diameter,RV transverse diameter,SV and PASP between the two groups(P＜0.05).The average length of hospital stay(7.24 d and 6.18 d),the average hospitalization cost(17 079.3 yuan and 13 369.2 yuan)and the western medicine cost(4 123.1 yuan and 3 037.1 yuan)were compared between the COPD high PASP group and the COPD normal PASP group.The difference was statistically significant(t=2.008,2.187,2.282,P＜0.05).Length of stay,RA inner diameter and transverse diameter,NT-proBNP were correlated with hospitalization costs(r=0.633,0.256,0.492,P＜0.01).Multiple stepwise regression analysis showed that high mean length of stay,high NT-proBNP,low SV and high RA diameter were risk factors for increased hospitalization costs in COPD patients.Conclusion Pulmonary hypertension will prolong the hospitalization days of COPD patients,increase the hospitalization expenses,increase the disease burden,and affect the social and economic benefits,which should be paid great attention by respiratory doctors.关键词
