俯卧位通气联合布地奈德应用于新生儿呼吸窘迫综合征的临床观察
Clinical Observation on Prone Position Ventilation Combined with Budesonide in the Treatment of Neonatal Respiratory Distress Syndrome

Abstract
Objective:To investigate the application effect of prone position ventilation combined with Budesonide in the treatment of neonatal respiratory distress syndrome(NRDS).Method:A total of 62 neonates with NRDS who were treated in the Neonatal Intensive Care Unit of Jiujiang Maternal and Child Health Hospital from December 2021 to January 2023 were selected and divided into the experimental group(31 cases)and the conventional group(31 cases)by random number table method.The conventional group was treated with continuous positive airway pressure(CPAP)assisted ventilation,and the experimental group was treated with prone position ventilation combined with Budesonide on this basis.The rapeutic effects in the two groups were evaluated,and the blood gas indexes,clinical indicators and the incidence of complications were compared between the two groups.Result:After treatment,the total effective rate of the experimental group was higher than that of the conventional group;the oxygen index(OI)of the experimental group was lower than that of the conventional group,and the pH value and arterial partial pressure of oxygen(PaO2)were higher than those of the conventional group,the differences were statistically significant(P<0.05).During the treatment,the times of exogenous pulmonary surfactant(PS)use in the experimental group were less than those in the conventional group,and the time of oxygen use,hospitalization time and CPAP use in the experimental group were shorter than those in the conventional group,the differences were statistically significant(P<0.05).The total incidence rate of adverse reactions such as pulmonary hypertension,pneumothorax,gastrointestinal bleeding,feeding intolerance,hip pressure sore,intracranial hemorrhage and conversion to invasive ventilator in the experimental was lower than that in the conventional group,the difference was statistically significant(P<0.05).Conclusion:Prone position ventilation combined with Budesonide has a significant effect in the treatment of NRDS in children,which can significantly improve the respiratory function of children,shorten the time of oxygen use and hospital stay,with good safety.关键词
