Effect of Proton Pump Inhibitors on the Efficacy of Immune Checkpoint Inhibitors

Abstract
Objective To ensure the efficacy of immune checkpoint inhibitors(ICIs)in cancer patients.Methods A total of 71 cancer patients treated with ICIs admitted to the Jiading District Central Hospital Affiliated to Shanghai University of Medicine&Health Sciences from January 2021 to December 2022 were selected and divided into the PPI group(49 cases)and the Non-PPI group(22 cases)based on whether they had used PPIs no less than 3 d within 30 d before and after ICIs treatment.The univariate and multivariate analysis methods were used to explore the correlation between the use of PPIs and related clinical indications of patients,and the Kaplan-Meier survival curve was used to analyze the effect of PPIs use on the efficacy.Results The disease control rate in the PPI group and the Non-PPI group was similar(75.51%vs.77.27%,P = 0.872).The Kaplan-Meier survival curve showed that the average progression-free-survival(PFS)and overall survival(OS)in the Non-PPI group was 20.533 and 20.629 months respectively,which was significantly longer than 12.282 and 14.182 months in the PPI group(P = 0.038,0.034)respectively.The univariate analysis showed that the use of PPIs had a borderline effect on patients'PFS[HR = 3.369,95%CI(0.991,11.455),P = 0.052],and could significantly affect the OS[HR = 3.466,95%CI(1.018,11.799),P = 0.047].The multivariate analysis showed that the use of PPIs was an independent influencing factor for the PFS[HR = 3.632,95%CI(1.045,12.618),P = 0.042]and OS[HR = 3.803,95%CI(1.068,13.541),P = 0.039].The average PFS and OS of lung cancer patients in the two groups was similar(P>0.05).Conclusion For cancer patients permanently residing in China,the long-term use of PPIs(no less than 3 d within 30 d before and after ICIs treatment)can reduce the efficacy of ICIs,shorten the PFS and OS of patients with solid tumors.关键词
