ECP/MPO、HMGB1与鼻内窥镜微创手术患者术后症状控制效果的关系
Analysis of the relationship between ECP/MPO,HMGB1 and postoperative symptom control in patients with minimally invasive nasal endoscopic surgery

Abstract
Objective To analyze the relationship between eosinophil cationic protein(ECP)/myeloperoxidase(MPO),high mobility group protein B1(HMGB1)and postoperative symptom control in patients undergoing minimally invasive nasal endoscopic surgery.Methods A total of 274 patients with chronic sinusitis who visited the Otolaryngology Clinic at Xinjiang Uygur Autonomous Region People's Hospital from August 2020 to April 2022 were selected.Among them,137 patients were randomly assigned to the training group,and the remaining 137 patients were assigned to the vali-dation group.All patients underwent endoscopic minimally invasive surgery for treatment.The ECP/MPO and HMGB1 levels were compared between the training group before surgery and 7 days after surgery,and the symptom control status was recorded 3 months after surgery.The factors influencing the postoperative symptom control in the training group were analyzed,and receiver operating characteristic(ROC)curves were plotted to evaluate the predictive value of ECP/MPO and HMGB1 for postoperative symptom control in chronic sinusitis patients in the training group.Decision curve analysis(DCA)was also conducted.The predictive ability of the indicators was evaluated using data from the validation group.Results The ECP/MPO and HMGB1 levels in the training group were lower 7 days after surgery than before surgery(P<0.05).Binary logistic regression analysis showed that 7-day postoperative Lund-Kennedy score,total visual ana-log scale(VAS)score for five subjective symptoms,ECP/MPO,and HMGB1 levels were independent factors influencing the postoperative symptom control in patients with chronic sinusitis treated by endoscopic minimally invasive surgery(P<0.05).The AUC value of ECP/MPO and HMGB1 combined detection in the training group was higher than that of ECP/MPO and HMGB1 alone(x2=3.036,2.976,P=0.002,0.003),and the maximum net benefit was 0.84.When the risk threshold was set at 0.10-0.72,the net benefit of ECP/MPO and HMGB1 combined detection was higher than that of single detection.In the validation group,the sensitivity and specificity of ECP/MPO and HMGB1 combined prediction for symptom control were 79.12%and 76.09%,respectively,the negative predictive value was 64.81%,and the posi-tive predictive value was 86.75%.Conclusion The expression of ECP/MPO and HMGB1 in patients undergoing endo-scopic minimally invasive surgery is closely related to the postoperative symptom control.Early postoperative detection of their levels may provide reference for clinical prediction and prognosis.关键词
