Abstract
Objective To analyze pregnancy and delivery outcomes of confined placental mosaicism(CPM).Methods Clinical data of prenatal diagnosis and placental chromosome testing were collected from abnormal samples ob-tained during non-invasive prenatal testing(NIPT)from October 2017 to November 2021.The outcomes of pregnancies with CPM were analyzed.Results Among 48 samples that underwent placental chromosome testing(27 in the control group and 21 in the CPM group),5 cases(18.52％)in the control group had complications during pregnancy,including 1 case of preeclampsia,2 cases of gestational diabetes mellitus(GDM),1 case of gestational hypertension,and 1 case of thrombocytopenia with edema.In the CPM group,6 cases(30.00％)had complications during pregnancy,including 4 cases of GDM,1 case of severe preeclampsia,and 1 case of thrombotic pre-state.In the control group,2 cases(7.41％)of fetal ultrasound abnormalities were detected,including 1 case of mild separation of the fetal right kidney(7.2 mm)and 1 case of oligohydramnios,while the remaining 25 cases showed no fetal ultrasound abnormalities,and all 27 newborns were delivered successfully.In the CPM group,4 cases(20.00％)of fetal ultrasound abnormalities were de-tected,including 1 case with fetal ultrasound suggesting polydactyly of the right hand,and 3 cases with decreased amniotic fluid,among which 1 case was complicated by fetal growth restriction(FGR).One case showed a possibly pathogenic de-letion in the 21q22.3 region of the amniotic fluid.The mother insisted on inducing labor,and the other 20 newborns were delivered successfully.In the control group,2 cases(7.41％)of small for gestational age(SGA)were observed,with birth weight Z-Scores of-3.01SD and-4.63SD,respectively.In the CPM group,4 cases(20％)of SGA were ob-served,including 1 case of Trisomy 16(birth weight Z-Score:-2.61SD),1 case of Trisomy 13(birth weight Z-Score:-3.29SD),and 2 cases with mosaic X monosomy(birth weight Z-Scores:-2.79SD,-3.47SD).Conclu-sion The incidence of pregnancy complications,fetal ultrasound abnormalities,and SGA in the CPM group were higher than those in the control group.关键词
