超声刀在鼻内镜下保留舌骨的甲状舌管囊肿切除术中的应用
Application of ultrasonic scalpel in thyroglossal duct cyst resection with hyoid bone preserved under nasal endo-scope

Abstract
Objective To evaluate the application of ultrasonic scalpel in endoscopic resection of thyroglossal duct cysts(TDC)with preservation of the hyoid bone.Methods Fifty-four pediatric patients with TDC who visited the Department of Otolaryngology-Head and Neck Surgery at the Fifth Affiliated Hospital of Xinjiang Medical University from May 2020 to October 2022,were selected and randomly divided into an observation group and a control group each(n=27).Both groups underwent endoscopic resection of thyroglossal duct cysts with preservation of the hyoid bone.The con-trol group used a high-frequency electric knife for hemostasis during the surgery,while the observation group used an ul-trasonic scalpel for hemostasis.The treatment effect,operation time,intraoperative blood loss,postoperative drainage vol-ume,hospitalization time,postoperative pain level,incidence of complications,pre-and postoperative stress response indicators,and vocal assessment were compared between the two groups.Results Complete resection of cysts and fistulas was achieved in both groups with good results.The operation time in the observation group was significantly shorter than that in the control group,and the intraoperative blood loss was significantly less than that in the control group(P＜0.05).The postoperative pain level in the observation group was lower than that in the control group within 24 hours after surgery(P＜0.05).The levels of blood glucose(GLU),cortisol(COR),and norepinephrine(NE)in both groups increased after 1 day of surgery,but the observation group was significantly lower than the control group(P＜0.05).After 1 day of surgery,the values of jitter and shimmer increased,while the value of normalized noise energy(NNE)decreased com-pared to preoperative values in both groups,but the magnitude of change in the observation group was significantly smaller than that in the control group(P＜0.05).The incidence of complications was not statistically different between the two groups(P＞0.05).Conclusion The application of an ultrasonic scalpel in endoscopic resection of thyroglossal duct cysts with preservation of the hyoid bone can shorten the operation time,reduce intraoperative blood loss,alleviate postop-erative pain and stress response,and minimize the impact on vocal function after surgery in pediatric patients with TDC.It is a safe and effective treatment approach.关键词
