Abstract
Objective To investigate the feasibility and safety of Estradiol Tablets/Estradiol and Dydrogesterone Tablets combined with hysteroscopic adhesion cold knife separation in the treatment of severe intrauterine adhesions.Methods A total of 80 patients with severe intrauterine adhesion admitted to Maternity & Child Care Center of Xinyu from January 2020 to September 2021 were selected as the research objects,and they were divided into observation group(40 cases)and control group(40 cases)according to random number table method.The control group was treated with hysteroscopic adhesion cold knife separation+Estradiol Valerate Tablets+Dydrogesterone Tablets,on the basis of which,the observation group was treated with Estradiol Tablets/Estradiol and Dydrogesterone Tablets in combination.Both groups were treated for 1 month and followed up for 12 months.The clinical effects,endometrial status and menstrual recovery,uterine arterial hemodynam-ics,serum matrix metalloproteinase-9(MMP-9),transforming growth factor-β(TGF-β)levels,readhesion rate and safety were compared between the two groups.Results After treatment,the total clinical effective rate of observation group was higher than that of control group,the difference was statistically significant(P＜0.05).After treatment,the endometrial thickness in both groups were thicker than those before treatment,and the endometrial thickness in observation group was thicker than that of control group,the menstrual volume in both groups were higher than those before treatment,and the menstrual volume in the observation group was high-er than that in the control group,the differences were statistically significant(P＜0.05).After treatment,the levels of pul-satility index(PI),uterine artery resistance index(RI)and serum transforming growth factor(TGF)-βin two groups were lower than those before treatment,and all indexes in observation group were lower than those of control group,with statistical significances(P＜0.05).After treatment,blood flow index(FI)and serum matrix metalloproteinase-9(MMP-9)levels in two groups were higher than those before treatment,and FI and MMP-9 levels in observation group were higher than those in control group,with statistical significances(P＜0.05).The readhesion rate of observation group(5.00％)was lower than that of control group(20.00％),and the difference was statistically significant(P＜0.05).There was no significant difference in the proportion of endometrial types between the two groups(P＞0.05).Conclusion Estradiol Tablets/Estradiol and Dydro-gesterone Tablets combined with hysteroscopic adhesion cold knife separation in the treatment of severe intrauterine adhe-sion can promote endometrial repair and improve menstrual status,and can regulate the changes of serum TGF-β and MMP-9 levels,effectively prevent postoperative readhesion,and have high overall safety.关键词
