Abstract
Objective To analyze the clinicopathological characteristics and the prognostic risk factors of breast cancer patients with HER2-zero and HER2-low expression.Methods A total of 732 HER2-negative breast cancer patients admitted to the First Hospital of Shanxi Medical University from January 2018 to December 2021 were collected and divided into HER2-low group and HER2-zero expression group.The clinicopathological characteristics and the prognosis of patients were analyzed in the two groups.Kaplan-Meier curve was used to analyze the survival of patients,and Cox proportional risk regression model was used to analyze the influencing factors of prognosis.Results There were 441 cases(60.25％)of HER2-low expression and 291 cases(39.75％)of HER2-zero expression.Compared with HER2-zero expression group,the proportion of non-specific histological types,the lymph node metastasis rate,the positive rates of HR and AR were increased in HER2-low expression group(P＜0.01),and the histological grade was decreased(P＜0.001).Survival analysis showed that the overall survival(OS)in HER2-zero expression group was significantly lower than that in HER2-low expression group(P=0.016),while there was no significant difference in disease-free survival(DFS)between the two groups(P=0.534).There were no significant differences in DFS and OS between hormone negative patients and positive patients(P＞0.05).Higher age at diagnosis,higher histological grade(G3 vs G1-2),vascular invasion,lymph node metastasis,higher proliferation index of Ki67(≥14％vs＜14％)were independent risk factors affecting DFS and OS in patients(P＜0.05).HER2-zero expression versus HER2-low expression was also an independent risk factor for patients'OS(HR=0.393,95％CI 0.228-0.679,P=0.001).Conclusion The positive rate of the hormone receptor is higher in HER2-low expression patients,but the survival status(DFS and OS)is not related to the hormone status.Higher age of diagnosis,histological grade,Ki67 proliferation index and vascular invasion,lymph node metastasis are independent risk factors affecting DFS and OS in patients.关键词
