早期IL-12p70、CAR、PCT联合检测对急性胰腺炎病情严重程度的预测价值
Predictive Value of Early Combined Detection of IL-12p70,CAR and PCT for the Severity of Acute Pancreatitis

Abstract
Objective To investigate the correlation between the early combined detection of interleukin 12p70(IL-12p70),C-reactive protein/albumin ratio(CAR)and procalcitonin(PCT)and the severity of acute pancreatitis(AP)and its early predictive efficacy for non-mild pancreatitis.Methods The clinical data of 102 patients with AP admitted to the emergency surgery department of our hospital from November 2021 to May 2022 were retrospectively analyzed.According to the revised Atlanta classification,they were divided into mild group(48 patients)and non-mild group(54 patients).The levels of IL-12p70,CAR and PCT at admission were compared between the two groups.The receiver operating curve(ROC)and correlation analysis were used to explore the predictive efficacy of early combined detection of the above indicators on the severity of AP.Results There was no significant difference in gender,cause of disease,age,blood pressure,BMI,S-AMY,Ca2+and WBC between the two groups(P＞0.05).The levels of IL-12 p70,CAR and PCT in the non-mild group were higher than those in the mild group(P＜0.05).The ROC curve showed that the areas under the curve of IL-12p70,CAR,and PCT for early prediction of non-mild AP were 0.866,0.762,and 0.927,respectively.Parallel detection of the three could increase the sensitivity of the detection to 0.996,and the specificity was 0.998.Correlation analysis showed that early IL-12p70,CAR and PCT were positively correlated with modified CT severity index score(MCTSI)(r=0.603,0.420,0.669).Conclusion Early detection of IL-12p70,CAR and PCT has certain clinical value in predicting the severity of AP patients,and the combined use of the three methods has a better predictive effect.关键词
