皮肤镜在头皮银屑病与头皮脂溢性皮炎鉴别诊断中的作用

Abstract
Objective To compare the dermoscopic characteristics of scalp psoriasis and seborrheic dermatitis for differential diagnosis.Methods Thirty-three patients with scalp psoriasis(SP group)and fourty-five patients with scalp seborrheic dermatitis(SSD group)were selected.And ninety-five patients involved in other parts of the SP group and fifteen patients involved other parts of the SSD were selected.The dermatoscopic characteristics of skin lesions in the four groups were compared and the indicators studied were scale color,vascular arrangement pattern and vessel morphology.Results The most prominent dermoscopic feature in the SP group was white/silver white scales(87.9%),regular red dots/spherical vessels(95.7%)at low magnification,glomerular vessels(87.6%),twisted/hairpin vessels(84.8%)at high magnification.The SSD group mainly showed the yellow scales(80.0%),branched vessels(86.9%)and atypical vessels(91.1%).The dermoscopic features differences were statistically significant(P<0.05).The most prominent feature of dermoscopy in the otherwise involved SP group was white/silver white scales(91.6%),regular red dots/spherical vessels(93.7%)at low magnification,glomerular vessels(90.5%),circular/hairpin vessels(88.4%)at high magnification.The SSD group involving other sites consisted mainly the yellow scales(73.3%),branch vessels(73.3%),and atypical vessels(86.7%).The dermoscopic features differences were statistically significant(P<0.05).Conclusions Dermoscopy is a practical and noninvasive method to assist in the identification of scalp psoriasis and seborrheic dermatitis.关键词
