单侧双通道内镜入路与开放入路椎板开窗髓核摘除术治疗腰椎间盘突出症的近期效果
Short-term Effects of Unilateral Biportal Endoscopic Approach and Open Approach of Lumbar Fenestration Decompression in the Treatment of Lumbar Disc Herniation

Abstract
Objective:To investigate the short-term effects of two lumbar fenestration decompression(LFD)approaches for the treatment of lumbar disc herniation.Method:A total of 45 patients with single-stage simple lumbar disc herniation admitted to the Spine Surgery Department of Fuzhou Second Hospital from October 1,2020 to June 30,2022 were included and divided into observation group and control group according to the surgical method.The observation group(n=25)underwent unilateral biportal endoscopic(UBE)approach LFD,and the control group(n=20)underwent open approach LFD.The surgical indexes,imaging indexes,serology indexes,lumbar and leg visual analogue scale(VAS),Oswestry disability index(ODI),curative effect and complications were compared between the two groups.Result:There were no related complications such as nerve and blood vessel injury and spinal dura mater avulsion after surgery in both groups.There was no significant difference in operation time between the two groups(P>0.05).The intraoperative bleeding volume in the observation group was less than that in the control group,and the length of postoperative hospital stay and incision length were shorter than those in the control group(P<0.05).There were no significant differences in creatine kinase,hemoglobin,hematocrit,lumbar lordosis,segmental lumbar lordosis,lumbar lordosis index and ODI between the two groups(P>0.05).The lumbar and leg VAS score of the observation group were significantly lower than those of the control group at all time points after surgery(P<0.05).The excellent and good rate was 92.00%in the observation group and 90.00%in the control group,there was no statistical significance between the two groups(P>0.05).Conclusion:The efficacy of the two approaches to LFD is similar,but the UBE approach IFD have less intraoperative bleeding volume,lower lumbar and leg VAS,and shorter postoperative hospital stay.关键词
