基于2019版Beers标准评价某三级甲等医院老年2型糖尿病患者潜在不适当用药情况
Evaluation of Potentially Inappropriate Medications in Elderly Patients with Type 2 Diabetes Mellitus in a Tertiary Hospital Based on the 2019 Edition of Beers Criteria

Abstract
Objective To promote rational drug use in elderly patients with type 2 diabetes mellitus(T2DM).Methods A retrospective analysis was conducted on the clinical data of 527 T2DM patients(≥ 65 years old)admitted to the endocrine department of the hospital from January to December 2021,including their gender,age,severity of the condition,number of diseases,type of medication,payment method,and length of hospital stay.Based on the 2019 Edition of the Beers Criteria,potentially inappropriate medication(PIM)in medical records was evaluated,and the risk factors leading to PIM were determined through the Logistic regression analysis.Results A total of 527 medical records were included,with a male-to-female ratio of 1∶1.21.There were 504 cases(95.64％)aged 65-84 years old,456 cases(86.53％)with more than five types of diseases,461 cases(87.48％)with average severity,392 cases(74.38％)with more than 10 types of drugs,499 cases(94.69％)with medical insurance as the payment method,and 480 cases(91.08％)with 2-20 d of hospital stay.A total of 378 cases of PIM occurred in 289 patients(54.84％),including 188 cases(49.74％)of PIM in elderly T2DM patients,96 cases(25.40％)of PIM in elderly T2DM patients who should use drugs with caution,45 cases(11.90％)of PIM related to drug-drug interactions should be avoided in elderly T2DM patients,11 cases(2.91％)of PIM related to disease in elderly T2DM patients,38 cases(10.05％)of PIM related to drugs that should be avoided or reduced in patients with renal insufficiency.The results of Logistic regression analysis showed that the incidence of PIM increased with the age,number of diseases,severity of the condition,type of drugs,and length of hospital stay in elderly T2DM patients(OR＞1,P＜0.05).Conclusion The frequency of PIM in elderly T2DM patients in this hospital is relatively high,and physicians,pharmacists,and nurses need to strengthen cooperation to further improve the level of rational drug use.关键词
