从心支别络痛论治冠脉微血管病变发微
Discussion on the Treatment of Coronary Microvascular Disease from the Perspective of Heart Collateral Pain
摘要
Abstract
Coronary microvascular disease is a series of clinical syndromes caused by abnormal structure and/or function of coronary microvasculature,with angina pectoris as the main clinical manifestation,which falls into the category of"heartache"in traditional Chinese medicine.Heart collateral pain is a kind of traditional Chinese medicine heart pain disease in the small collaterals.This article analyzes the similarities in structure and physiological function between small collaterals and micro vessels,and believes that coronary microvascular disease can be treated from the perspective of heart collateral pain.By studying and sorting out the ancient books and documents of traditional Chinese medicine on collaterals,it is considered that the syndrome of heart collateral pain can be divided into deficiency and excess.The excess syndromes are mostly caused by wind pathogen and phlegm-retained fluid,or qi stagnation and blood stasis;and the deficiency ones are usually caused by weakness of the triple energizers.Its pathogenesis is the invasion of pathogens into the heart collaterals,which causes the obstruction of the collaterals,resulting in pain.Therefore,clinically,we can take freeing collateral to relieve pain as the therapeutic principles,and employ the following three kinds of treatment:dispelling wind and dissolving phlegm,and freeing collaterals to relieve pain,and modified combination of trichosanthes,Chinese chive and pinellia decoction and two matured substances decoction can be used;regulating qi and activating blood,and freeing collateral to relieve pain,and modified combination of immature bitter orange,Chinese chive and cinnamon twig decoction and sudden smile powder can be employed;fortifying spleen and replenish qi,and freeing collateral to relieve pain,and modified spleen-restoring decoction can be used.关键词
