新生儿早期肺部超声评分与新生儿急性生理学评分围生期补充-Ⅱ相关性的研究
Correlation between lung ultrasound score and SNAPPE-Ⅱ score in evaluating severity of early neonate lung diseases

Abstract
Objective To explore correlation between lung ultrasound score(LUS)and score for neonatal acute physiology with perinatal extension Ⅱ(SNAPPE-Ⅱ)in evaluating the severity of early neonate lung diseases.Methods A total of 98 newborns admitted to neonatal intensive care unit(NICU)due to pulmonary diseases in Ningbo Medical Treatment Center Lihuili Hospital from February 2021 to December 2022 were enrolled in the study.All the newborns were examined with lung ultrasonography 2 h after birth and scored,and also scored with SNAPPE-Ⅱ.According to the outcome,newborns were divided into improved group(n=87)and progressive group(n=11),and the LUS and SNAPPE-Ⅱ score were compared between two groups.The correlation between LUS and SNAPPE-Ⅱ was analyzed,and the accuracy of LUS and SNAPPE-Ⅱ score in predicting the severity of neonatal pulmonary diseases was analyzed with ROC curve.Results The proportion of newborns with weight ≥1 500 g and temporary tachypnea in the improved group was significantly higher than that in the progressive group(P＜0.05).The LUS and SNAPPE-Ⅱ score of the progressive group were significantly higher than those of the improved group(P＜0.01).There was a significant positive correlation between LUS and SNAPPE-Ⅱ(r=0.473,P＜0.05).The area under ROC curve(AUC)of LUS and SNAPPE-Ⅱ score for predicting the severity of neonatal lung disease was 0.911 and 0.928,respectively(P＞0.05).LUS＞6.20 and SNAPPE-Ⅱ＞13.31 were taken as cut-off values,the sensitivity and specificity of LUS were 0.85 and 0.91,and those of SNAPPE-Ⅱ score were 0.88 and 0.95,respectively(P＞0.05).Conclusion LUS is positively correlated with SNAPPE-Ⅱ,indicating LUS can be used for predicting the severity of early neonatal pulmonary disease.The best cut-off value of neonatal LUS was＞6.20 in this study.关键词
