局部应用氨甲环酸联合磺胺嘧啶银脂质水胶敷料对骨科大手术后切口渗血的影响
Influence on topical application of Tranexamic Acid combined with Sulfa-diazine Silver Lipid Hydrogel Dressing on incision bleeding after major orthopedic surgery

Abstract
Objective To investigate the influence on topical application of Tranexamic Acid combined with Sulfadiazine Silver Lipid Hydrogel Dressing on incision bleeding after major orthopedic surgery.Methods A total of 114 patients under-going major orthopedic surgery at Department of Orthopedics,Deyang People's Hospital from January 2022 to January 2023 were selected.They were divided into group A(38 cases),group B(38 cases)and group C(38 cases)by using random num-ber table method.Sulfadiazine Silver Lipid Hydrogel Dressing for external application was used in group A,5%Tranexamic Acid wet yarn wet application was applied in group B,and combined external application plan was used in group C.The de-gree of bleeding,pain during dressing changes,incision conditions and patient satisfaction among three groups were com-pared.Results The degree of bleeding in group C was milder than that in group A and group B,and the differences were statistically significant(P＜0.05).There were statistically significant differences in visual analogue scale(VAS)among three groups in terms of groups,time point and interaction effects(P＜0.05).VAS scores of the three groups during and after dressing change were higher than those before dressing change,while VAS scores of the three groups after dressing change were lower than those during dressing change,the differences were statistically significant(P＜0.05).VAS scores during and after dressing change in group C were lower than those in group A and group B,the differences were statistically significant(P＜0.05).The total amount of bleeding in group C wounds within 24 hours was less than that in group A and group B,the differences were statistically significant(P＜0.05).The purulent secretions,wound edema,granulation color,gran-ulation bleeding and granulation growth scores in group C were higher than those in group A and group B,the differences were statistically significant(P＜0.05).The satisfaction rate of group C was higher than that of group A and group B,the differences were statistically significant(P＜0.017).Conclu-sion The combination of external application of Tranexamic Acid and Sulfadiazine Silver Lipid Hydrogel Dressing can sig-nificantly reduce postoperative incision bleeding,and alleviate wound dressing pain in orthopedic surgery patients.关键词
