血浆NGAL、尿NAG及尿RBP在诊断早期糖尿病肾脏疾病中的价值
Value of plasma neutrophil gelatinase-associated lipid carrier protein,urinary N-acetyl-β-D-glucosaminidase and urinary retinol-binding protein in diagnosis of early diabetic kidney disease

Abstract
Objective To investigate the value of plasma neutrophil gelatinase-associated lipid carrier pro-tein(NGAL),urinary N-acetyl-β-D-glucaminoglycosidase(NAG)and urinary retinol-binding protein(RBP)in the diagnosis of early diabetic kidney disease(DKD).Methods A total of 227 patients with type 2 diabetes mellitus(T2DM)diagnosed in the department of endocrinology in Shanghai Pudong New Area Gongli Hospi-tal from May 2021 to March 2023 were selected as the study subjects and further divided into the normal albu-minuria(NA)group,microalbuminuria(MA)group and macroalbuminuria(HA)group according to the mi-croalbuminuria/creatinine ratio(UACR).The other 88 healthy subjects in physical examination were selected as the control(NC)group.The levels of UACR,NGAL,NAG,RBP,blood glucose and renal function were de-tected in each group.The correlation between NGAL,NAG and RBP with DKD was analyzed.The value of each index in diagnosing early DKD was analyzed by the receiver operating characteristic(ROC)curve.Results There were 97 cases in NA group,73 cases in MA group and 57 cases in HA group according to UACR level.The levels of NGAL,NAG and RBP in the NA group,MA group and HA group were higher than those in the NC group,moreover the HA group was>MA group>NA group,and the differences were statis-tically significant(P<0.05).The Pearson correlation analysis showed that the NGAL,NAG and RBP levels were positively correlated with UACR,BUN,SCr and CysC level(P<0.05).The ROC curve analysis showed that the area under the curve(AUC)of NGAL,NAG,RBP and their combined detection for diagnosing DKD were 0.854,0.788,0.830 and 0.929 respectively in turn.The sensitivity of NGAL single detection was higher(82.3%),and the diagnostic efficiency of NGAL,NAG and RBP combined detection was maximal(AUC=0.929).Conclusion The levels of plasma NGAL,urine NAG and RBP are closely related to the degree of kid-ney damage in DKD patients.Their combined detection could exert the maximum diagnostic efficiency and provide a new method for early diagnosis of DKD.关键词
