地氟烷和七氟烷对术后认知功能障碍影响的荟萃分析
Effect of desflurane and sevoflurane on postoperative cognitive dysfunction:a meta-analysis

Abstract
Objective:To evaluate the effects of desflurane and sevoflurane on postoperative cognitive dysfunction.Methods:Randomized controlled trial studies on the effects of intraoperative maintenance of general anesthesia with desflurane and sevoflurane on patients'postoperative cognitive dysfunction were collected through a computerized search of PubMed,Embase,Cochrane Library,China National Knowledge Infrastructure,Wanfang Database and Wipro Chinese Journal Database from the year of the library's establishment to May 2023.Papers were screened,extracted and evaluated for quality according to the inclusion and exclusion criteria,and the included papers were analyzed using RevMan 5.3 and Stata 15.1 software.Results:A total of 13 papers were included,including 1,046 patients,of which 523 were treated with intraoperative desflurane and 523 with sevoflurane.The results of meta-analysis showed that the mini-mental state examination scores of the desflurane group were higher than those of the sevoflurane group at 1 hour after surgery(SMD=1.65,95%CI:0.70-2.60,P<0.001),3 hours after surgery(SMD=0.45,95%CI:0.13-0.76,P=0.006),6 hours after surgery(SMD=1.15,95%CI:0.18-2.12,P= 0.02),12 hours after surgery(SMD=0.94,95%CI:0.18-1.70,P=0.01),and 24 hours after surgery(SMD=0.20,95%CI:0.06-0.33,P=0.005).The time to open the eyes in the desflurane group(SMD=-2.95,95%CI:-4.42--1.47,P<0.001)was shorter than in the sevoflurane group.Conclusion:Desflurane is more effective in reducing postoperative cognitive dysfunction and has a shorter awakening time than sevoflurane.关键词
