Abstract
Objective:To investigate the prognostic value of systemic immune-inflammation index(SII)combined with tumor TNM stage and age in patients undergoing radical esophagectomy.Methods:The clinical data of 150 patients who underwent radical esophagectomy in Jieyang People's Hospital from 2014 to 2020 were retrospectively analyzed,including 103 males and 47 females,aged(62.1±6.7)years.The optimal cut-off value of SII was determined using receiver operating characteristic(ROC)curve,and the patients were divided into pre-treatment high SII group,pre-treatment low SII group and follow-up high SII group and follow-up low SII group.The Kaplan-Meier method was used to draw survival curves,and the log-rank test was used for one-way survival analysis,and the Cox proportional risk regression model was used for multivariate survival analysis and to construct prognostic index equations.The prognostic index equation was evaluated using ROC curve and calibration curve assessment.Results:The optimal cut-off value of pre-treatment SII was determined to be 782.5×109 based on the ROC curve,with 44 cases in the pre-treatment high SII group(SII≥782.5×109)and 106 cases in the pre-treatment low SII group(SII<782.5×109).The optimal threshold for the mean value of follow-up SII was determined to be 729.5×109 based on the ROC curve,with 65 cases in the follow-up high SII group(SII≥729.5×109)and 85 cases in the follow-up low SII group(SII<729.5×109).Cox regression analysis showed pretreatment SII(HR=2.339,95%CI=1.443-3.790),follow-up SII(HR=4.509,95%CI=2.620-7.759),age,and tumor TNM stage were the independent influencing factors of patients'progression-free survival rate.Prognostic index = 0.850×pretreatment SII grouping + 1.506×follow-up SII grouping + 0.035×age + 1.561×TNM stage Ⅱ + 2.136×TNM stage Ⅲ + 3.530×TNM stage Ⅳ.The C-index of the prognostic equation was 0.811(95%CI:0.762-0.859),and the calibration curve was close to the diagonal,suggesting that the prognostic index equation was accurate and well calibrated.Conclusion:SII combined with tumor TNM stage and age has a better prognostic value for patients with squamous esophageal cancer undergoing radical esophagectomy.关键词
