紧密型县域医共体内患者基层首诊意愿及影响因素研究
Patients' willingness of the first-visit at grassroots medical institution and the influencing factors in a compact county medical community

Abstract
Objective Taking Xinyi,Jiangsu Province,a national-level pilot area as an example,to explore the promotion effect of the first-visit at grassroots medical institution in compact county medical community,so as to provide reference for promoting the development of the compact county medical community and the first-visit sys-tem at the grassroots level.Methods A questionnaire survey was conducted among 1068 patients seeking medical care in the compact county medical community.Random forest and logistic regression were adopted to analyze the influencing factors of patients'willingness for first-visit.Results 80.1％of the patients were willing to pay the first-visit at the grassroots level.The influencing factors for patients first-visit at the grassroots level are age,type of registered permanent residence,distance from home to medical institution,type of medical institution,whether having a contracted family doctor or not,and whether being accessible to two-way referral or not.Conclusions Patients'willingness of first-visit at grassroots medical institution in the compact county medical community in Xi-nyi is relatively high.It is recommended to improve the service capabilities of primary medical institutions,pro-mote resource sharing within the medical community,strengthen the construction of information systems,and pro-mote the signing of family doctors.关键词
