Abstract
Objective To investigate the clinical significance of serum soluble intercellular adhesion molecule-1(sICAM-1),soluble vascular cell adhesion molecule-1(sVCAM-1)and endothelin-1(ET-1)in Henoch-Schoenlein purpura(HSP).Methods A total of 85 children with HSP admitted to Quanzhou Women's and Children's Hospital from May 2020 to May 2022 were retrospectively selected,and the levels of serum sICAM-1,sVCAM-1 and ET-1 were detected at admission.They were divided into the Henoch-Schoenlein purpura nephritis(HSPN)group(n=28)and the non-HSPN(nHSPN)group(n=57)according to whether there was renal involvement,and were divided into the non-recurrent group(n=69)and the recurrent group(n=16)according to whether there was recurrence after treatment.In addition,children who underwent physical examination in the same period were selected as the healthy control group(n=30).Results Compared with the healthy control group,the levels of serum sICAM-1,sVCAM-1 and ET-1 in the nHSPN group and the HSPN group all increased remarkably,with statistically significant differences(P<0.05).Compared with the nHSPN group,the levels of serum sICAM-1,sVCAM-1 and ET-1 in the HSPN group also increased remarkably,with statistically significant differences(P<0.05).Correlation analysis showed that serum sICAM-1 was positively correlated with serum sVCAM-1(r=0.682,P<0.05),serum sICAM-1 was positively correlated with serum ET-1(r=0.536,P<0.05),and serum sVCAM-1 was positively correlated with serum ET-1(r=0.614,P<0.05).Compared with the non-recurrent group,the serum sICAM-1 in the recurrent group increased remarkably,with statistically significant difference(P<0.05),but there were no statistically significant differences in the levels of sVCAM-1 and ET-1 between the two groups(P>0.05).The receiver operating characteristic(ROC)curve analysis showed that serum sICAM-1,sVCAM-1 and ET-1 and the combination of the three in predicting HSPN were 0.745,0.839,0.668 and 0.904,respectively,obviously larger than the area under the opportunity reference line,with statistically significant differences(P<0.05).However,the AUC for predicting disease recurrence was 0.533,0.512,0.504 and 0.569,respectively,and there were no statistically significant differences compared with the area under the opportunity reference line(P>0.05).Conclusion SICAM-1,sVCAM-1 and ET-1 are involved in the occurrence and development of HSP,and can be used as potential markers to predict HSPN.关键词
