HER2低表达激素受体阳性早期乳腺癌患者临床病理特征及预后分析
Clinicopathological features and prognosis of early breast cancer patients with positive hormone receptor and low expression of HER2

Abstract
Objective To analyze the clinicopathological features and prognosis of early breast cancer patients with positive hormone receptor(HR)and low expression of human epidermal growth factor receptor 2(HER2).Methods A total of 498 HR positive breast cancer patients,who were treated in Shiyan Taihe Hospital between January 2014 and January 2021,were included in the analysis.According to immunohistochemistry results patients were divided into HER2-negative group[HER2(0)]and HER2-low group[HER2(+)or HER2(++)and fluorescence in situ hybridization negative].Clinicopa-thological characteristics and prognosis were compared between the two groups.Results There were no significant differences in age,tumor diameter,lymph node metastasis,histological grade,estrogen receptor and adjuvant treatment between the two groups(all P＞0.05).Compared with the HER2-negative group,the HER2-low group had higher proportion of patients with median percentage of ER positive expression≥80％,positive progesterone receptor expression and lower Ki-67 expression;and lower proportion of patients with invasive lobular cancer and vascular invasion(all P＜0.05).With a median follow up of 65 months,the difference in disease free survival and overall survival between the two groups was not statistically significant(P＞0.05).Multivariate analysis showed that HER2-low expression was an independent influencing factor for disease free survival(HR=0.698,95％CI:0.493-0.961,P＜0.05).Conclusion In HR positive breast cancer,patients with HER2-low expression has similar prognosis to HER2-negative,but HER2-low expression is an independent protective factor for disease-free survival of patients,which is associated with high positive hormone receptor,low Ki-67 expression and low vascular invasion.关键词
