血清视黄醇结合蛋白、胱抑素C及载脂蛋白B/载脂蛋白A1联合检测对高血压早期肾损伤的诊断价值
Diagnostic value of serum retinol-binding protein,cystatin C and apoli-poprotein B/Apolipoprotein A1 combined detection in early hypertensive renal injury

Abstract
Objective:To investigate the diagnostic value of retinol-binding protein(RBP),cystatin C(Cys-C)and APolipopro-tein B/APolipoprotein A1(ApoB/ApoA1)in the early renal injury of hypertension.Methods:Retrospective analysis of clinical data from 80 patients with hypertension,according to the diagnostic criteria of early renal injury,they were divided into simple hypertension group(n =47)and hypertension combined with early renal injury group(n =33).The general information,blood lipid,RBP,Cys C,ApoB/ApoA1 levels of the two groups were compared.The risk factors of early renal injury in hypertensive patients were analyzed by Logistic regression.ROC curve was used to evaluate the diagnostic efficacy of RBP,Cys C,ApoB/ApoA1 and their combination for early renal injury in patients with hypertension.Results:The levels of RBP,Cys-C and ApoB/ApoA1 in the early renal injury group were higher than those in the simple hypertension group,the level of ApoA1 was lower than that in the simple hypertension group,and the differences between the groups were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that RBP,Cys C,ApoB/ApoA1 were risk factors of early renal injury in patients with essential hypertension(P<0.05).ROC analysis showed that when RBP≥3.475 mg/L,the area under the curve(AUC)of diagnosing early renal injury in patients with essential hypertension was 0.729,the sensitivity and specificity were 69.7%and 72.3%respectively.When Cys C≥2.525 mg/L,the AUC was 0.768,the sensi-tivity was72.7%,and the specificity was74.5%.When ApoB/ApoA1≥0.870,the AUC was 0.656,the sensitivity was 63.6%,and the specificitywas68.1%.The AUC of the combined diagnosis was 0.870,the sensitivity was 81.8%,and the specificity was 80.9%.Conclusion:The levels of RBP,Cys C and ApoB/ApoA1 in patients with hypertension and early renal injury increased,which can be used in the auxiliary diagnosis of hypertension and early renal injury.关键词
