超声造影在甲状腺结节诊断及消融疗效评估中的应用价值
Application value of contrast-enhanced ultrasound in the diagnosis of thy-roid nodules and efficacy evaluation of ablation

Abstract
Objective:To explore the application value of contrast-enhanced ultrasound(CEUS)in the diagnosis of thyroid nod-ules and efficacy evaluation of ablation.Methods:90 patients with thyroid nodules were divided into benign group(n =72)and malig-nant group(n =18)according to pathological results,and all underwent CEUS examination and puncture biopsy.CEUS quantitative pa-rameters in the two groups were observed,and their diagnostic value for benign and malignant nodules was evaluated.The patients in be-nign group underwent microwave ablation(MWA)and then were divided into color Doppler flow imaging(CDFI)guidance group(n = 36)and CEUS guidance group(n =36).The nodule reduction rate,complete ablation and in situ recurrence rate were observed.Re-sults:The arrival time of contrast agent(IRF),time to peak(TTP)and area under the curve(AUC)in benign group were higher than those in malignant group(P<0.05).The results of ROC curve analysis showed that CEUS quantitative parameters were all of diagnos-tic value for benign and malignant thyroid nodules(P<0.05).At 1,6 and 12 months after surgery,reduction rates of nodules volume in CEUS guidance group were higher than those in CDFI guidance group(P<0.05).At 12 months after surgery,complete ablation rate in CEUS guidance group was higher than that in CDFI guidance group,and recurrence rate in situ was lower than that in CDFI guidance group(P<0.05).Conclusion:CEUS is not only can be applied in the differential diagnosis of benign and malignant thyroid nodules,but also can effectively evaluate ablation efficacy.关键词
