重庆某三甲医院住培医师心理健康状况与工作满意度的关系及睡眠质量的中介效应分析
Relationship between mental health status and job satisfaction and mediation effect of sleep quality among residents in a grade A tertiary hospital in Chongqing

Abstract
Objective This paper aims to understand the current status of job satisfaction of residents and analyze its relation-ship with mental health status and explore the mediating effect of sleep quality in mental health status and job satisfaction.Methods A cross-sectional questionnaire was used to investigate 442 residents in a university-affiliated teaching hospital in Chongqing,with the use of the 20-item General Health Questionnaire(GHQ-12)and job satisfaction questionnaire.Analy-ses were performed to analyze the correlation between sleep quality,mental health status and job satisfaction of residents,and to test the mediating effect of sleep quality between mental health status and job satisfaction.Results The overall score of job satisfaction of resident doctors was(3.91±0.66)points,and significant differences were observed in job satisfaction scores between residents with different educational backgrounds and sleep quality(P＜0.05).Pearson correlation analysis showed that a significant positive correlation was found between sleep quality and job satisfaction in residents(r=0.482,P＜0.05);a significant negative correlation was detected between self-affirmation,depression and anxiety dimensions and job satisfaction in GHQ-20(r=-0.407,-0.142,-0.245;all P＜0.05).Meanwhile,a significant negative correlation was found between sleep quality and self-affirmation,depression and anxiety dimensions in residents GHQ-20(r=-0.230,-0.254,-0.356;all P＜0.05).The results of regression analysis showed that self-affirmation had a significant negative predictive effect on job satisfaction(P＜0.05)and depression had a significant negative predictive effect on job sat-isfaction(P＜0.05)when controlling marriage,education,and training length.The explanatory ability of the regression model for job satisfaction of residents increased by 13.2％when sleep quality was included as a mediating variable in the model.The direct negative predictive effect of self-affirmation on job satisfaction remained significant(P＜0.05).The re-sults of the mediation effect test showed that the 95％CI of the path(mental health problem → sleep quality → job satisfac-tion)did not contain 0,verifying that sleep quality had a mediating effect between mental health status and job satisfaction.The indirect effect of mental health problem on job satisfaction was-0.164(95％CI:-0.242～-0.098,P＜0.001),which accounts for 65.60％of the overall effect,indicating that sleep quality had a partial mediating effect between mental health status and job satisfaction.Conclusion Mental health status of residents are important factors affecting job satisfac-tion and mental health status can further affect the job satisfaction of residents through sleep quality.It is suggested that de-partments at all levels should pay attention to the mental health status of residents,balance clinical and teaching learning time,emphasize ability training,and raise their work satisfaction.关键词
