术前控制营养状况评分对远端胆管癌患者根治术后短期并发症和长期结局的预测
Effect of the CONUT score for short-term complications and long-term outcomes after radical surgery for distal cholangiocarcinoma

Abstract
Objective To investigate the effect of the preoperative controlling nutritional status(CONUT)score for short-term complications and long-term prognosis of radical resection of distal cholangiocarcinoma.Methods We retrospectively analyzed 111 patients who underwent radical resection surgery for distal cholan-giocarcinoma in the Department of General Surgery at The First Hospital of Lanzhou University from January 2017 to September 2022.The patients were divided into two groups based on their CONUT scores:a high CO-NUT(≥4 points)group including 51 patients,and a low CONUT(<4 points)group of 60 patients.The differ-ences between the general clinicopathological data and postoperative short-term outcomes were compared be-tween the two.Cox regression and prognostic Kaplan-Meier survival curves were used to analyze the survival data of the patients,and to analyze the risk factors affecting the their overall survival and disease-free survival disease-free survival.Results A correlation was found between CONUT and postoperative complications,overall survival,and disease-free survival,and the differences in these correlations were statistically significant(P<0.05).Kaplan-Meier curves demonstrated a poorer prognosis in patients with a high CONUT score(P = 0.004 1).Multifactorial Logistic analysis showed that a high CONUT score and T stage were independent risk factors for complications after radical surgery in patients with distal cholangiocarcinoma.Multifactorial Cox regression showed that the CONUT score,pathological stage,the presence of obstructive jaundice before sur-gery,the degree of differentiation and peripheral tissue invasion were independent prognostic factors affecting the long-term outcome of patients having had radical surgery for distal cholangiocarcinoma.Conclusion Preop-erative CONUT score is an independent influence on short-term complications and long-term prognosis of distal cholangiocarcinoma patients after radical surgery,and a preoperative CONUT score of≥4 suggests a poor prognosis.The CONUT score can be used as an assessment index for patients after radical surgery for distal cholangiocarcinoma and is of clinical significance for individualized postoperative treatment.关键词
