改良Overlap吻合术在SiewertⅡ、Ⅲ型食管胃结合部腺癌全腹腔镜消化道重建中的临床应用价值
Clinical value of modified overlap anastomosis in total laparo-scopic digestive tract reconstruction of Siewert type Ⅱand Ⅲ esophageal and gastric adenocarcinoma

Abstract
Objective To explore the clinical application value of modified overlap anastomosis compared with traditional tubular anastomosis in the reconstruction of digestive tract in denocarcinoma of gastroesopha-geal junction for Siewert type Ⅱ and Ⅲ esophagogastric junction adenocarcinoma.Methods The clinical data of 125 patients with Siewert Ⅱ and Siewert Ⅲ adenocarcinoma of the esophagogastric junction admitted from January 2018 to October 2021 in Department of General surgery,The Second Hospital of Lanzhou University were retrospectively analyzed.All patients completed gastroesophageal junction + D2 lymph node dissection,50 underwent esophagojejunostomy with modified overlap anastomosis and 75 underwent circular tubular anastomosis.The differences in surgical-related and clinical outcome indicators of the two types of anastomosis were compared.Results There was no difference in statistics between the circular tubular anas-tomosis group and modified overlap anastomosis group in terms of postoperative ventilation time,first eating time and the number of node dissection and intraoperative blood loss,etc(P>0.05).In the overlap anastomo-sis group,compared with the circular tubular anastomosis group,the total operation time,digestive tract recon-struction time,postoperative complications,the total incidence of the disease(anastomotic bleeding)were statistically significant(P<0.05).Conclusion The modified overlap anastomosis can be performed under total laparoscopy,with good operation field,simple and feasible steps,less postoperative complications,high safety and clinical application value.关键词
