人工晶状体度数选择对先天性白内障术后眼部发育的影响
Influence of intraocular lens degree selection on ocular development after congenital cataract surgery

Abstract
Objective The effects of intraocular lens(IOL)choice on ocular development after congenital cataract surgery were evaluated by evaluating the best corrected visual acuity(BCVA),axis length(AL),and spherical equivalent(SE)after cataract surgery combined with under corrected IOL or full corrected IOL implantation.Methods A prospective study.A total of 20 patients(20 eyes)with binocular congenital cataract diagnosed in Weifang Ophthalmology Hospital from December 2014 to December 2017 were collected,aged 2～4 years old and the average age was 2.73±0.91.Cataract surgery combined with one-stage IOL implantation was performed in both eyes of all patients.All patients were enrolled with the right eye and divided into under corrected IOL group(group A,10 cases in 10 eyes)and full corrected IOL group(group B,10 cases in 10 eyes)according to the different IOL degrees implanted.Patients in group A were selected as IOL with the degree under corrected for 2～3 D;Patients in group B were selected as IOL with full degree correction,and meas-urement errors were eliminated by combining the refractive results of 1 week after the surgery.SE and AL at 1 week after the surgery were used as grouping records.The measures included SE and AL at 1、6、12、24 and 48 months postoperative-ly,and logMAR BCVA at follow-up at 48 months postoperatively.Results The average age of patients in group A was(2.88±1.14)years old,and that of patients in group B was(2.58±0.71)years old.There was no significant difference between the two groups(P＞0.05).The mean SE was(2.64±1.46)D in group A and(-0.64±0.87)D in group B at one week after the opera-tion.The mean AL at one week after operation of group A and group B was(21.34±1.35)mm and(22.68±0.82)mm respectively.The mean SE was(2.62±1.47)D in group A and(-0.65±0.78)D in group B at 1 month after the sur-gery(P＜0.05).The mean SE was(-1.89±1.49)D in group A and(-3.16±1.08)D in group B at 48 months after the surgery(P＜0.05).The postoperative SE change value of group A was(-4.51±1.55)D,and that of group B was(-2.56±1.19)D,the difference was statistically significant(P＜0.05).The mean AL was(21.31±1.34)mm in group A and(22.68±0.83)mm in group B(P＜0.05).At 48 months follow-up,AL in group A increased by(1.48± 0.55)mm on average,and AL in group B increased by(1.39±0.60)mm on average,with no statistical difference be-tween the two groups(P＞0.05).The logMAR visual acuity was(0.27±0.18)in group A and(0.28±0.27)in group B at 48 months after the operation,and there was no statistical difference between the two groups(P＞0.05).Conclusions For patients aged 2～4 who underwent binocular congenital cataract surgery combined with IOL implantation,there was no significant difference in BCVA after 48 months of follow-up,regardless of the under corrected IOL or the full corrected IOL.The prognosis of visual acuity was related to the complications of nystagmus and strabismus and postoperative visual training.In addition,all patients showed a trend of ocular axial growth and myopia drift after the surgery.关键词
