妊娠期女性人乳头瘤病毒感染与不良妊娠结局的关系
Association between Human Papillomavirus Infection in Gestational Women and Adverse Pregnancy Outcomes
摘要
Abstract
Objective:To investigate the current status of human papillomavirus(HPV)infection, distribution of HPV subtypes, and impact of HPV infection on pregnancy outcomes in gestational women,in order to provide rationale for the prevention of adverse pregnancy outcomes and the prevention and treatment of cancer. Methods:A total of 1,894 gestational women who registered in Haikou Hospital of the Maternal and Child Health from January 2020 to December 2022 were included as research subjects. The clinical data,HPV testing results, and pregnancy outcomes were retrospectively collected.HPV infection rates, distribution of HPV subtypes, and the impact of HPV infection on adverse pregnancy outcomes were analyzed. Results:The HPV infection rate of the research subjects was 31.47%.The single infection, superinfection and multiple infection rates of them were 18.69%,8.87%and 3.91%,respectively.The infection rate of high-risk HPV(HR-HPV)(14.68%)was significantly higher than that of low-risk HPV(LR-HPV)(4.01%)(χ2 = 127.148,P<0.001).HR-HPV with top 5 detection rates were HPV 16,HPV 52,HPV 56,HPV 58 and HPV 18,with infection rates of 5.02%,4.75%,2.27%,1.95%and 1.80%,respectively.LR-HPV with top 3 detection rates were HPV 81,HPV 6 and HPV 43,with infection rates of 2.59%,1.21%and 0.79%,respectively.The overall incidence of adverse pregnancy out-comes in HPV-infected pregnant women(26.17%)was significantly higher than that in non-infected pregnant women(5.47%)(χ2 = 165.981,P<0.001).Unconditional multivariate logistic regression analysis showed that HPV infection in pregnant women was an independent risk factor for adverse pregnancy outcomes(OR = 4.688).Conclusion:The infection rate of HPV in pregnant women is relatively high, and com-mon subtypes of HPV include HR-HPV 16,HR-HPV 52,and others.HPV infection during pregnancy is an independent risk factor for adverse pregnancy outcomes in pregnant women. We should strengthen HPV education and screening during pregnancy preparation, prepregnancy, and pregnancy to reduce the risks of adverse pregnancy outcomes and malignant tumors.关键词
