Abstract
Objective To investigate the clinical efficacy of sputum aspiration and lavage with fibrobronchoscope combined with Xuebijing in the treatment of severe pneumonia(SP)complicated with respiratory failure and the effects on levels of oxidative stress and inflammation.Methods A total of 98 patients with SP combined with respiratory failure who were admitted to Anyang Traditional Chinese Medicine Hospital from February 2020 to September 2022 were selected and divided into the control group(49 cases)and the study group(49 cases)according to the random number table.The patients in the control group were treated with sputum aspiration and lavage via bronchofibroscope,while those in the research group were treated with sputum aspiration and lavage via bronchofibroscope+Xuebijing treatment.The efficacy,clinical indicators(time to symptom relief,admission to ICU,and time to use antibacterial drugs),arterial blood gas analysis,inflammatory factors,and oxidative stress level of the two groups were compared.Results The effective rate of treatment in the study group(93.88%)was higher than that in the control group(79.59%),and the difference was statistically significant(P＜0.05).Compared with control group,the clinical symptoms(cough,lung rales,fever)relief,mechanical ventilation,antibacterial drug use and ICU stay time in study group were lower,and the difference was statistically significant(P＜0.05).After treatment,the arterial oxygen partial pressure(PaO2),arterial oxygen saturation(SaO2),and serum superoxide dismutase(SOD)levels in the study group were all higher than those in the control group,and the differences were statistically significant(P＜0.05).The levels of PaCO2 and serum tumor necrosis factor were also higher in the study group-α(TNF-α),Interleukin-6(IL-6),Interleukin-8(IL-8),C-reactive protein(CRP),malondialdehyde(MDA),and 8-isoprostaglandin F2 α I8-iso-PGF2 α)The levels were all lower than those of the control group,and the differences were statistically significant(P＜0.05).Conclusion Xuebijing adjuvant treatment of severe pneumonia combined with respiratory failure has a definite clinical effect,which can effectively shorten the time to alleviate the symptoms,help to improve blood gas indicators,and inhibit the body's oxidative stress and inflammatory reaction.关键词
