生长抑素联合吲哚美辛对内镜逆行胰胆管造影术后胰腺炎患者的临床研究

Abstract
Objective To observe the effect of somatostatin combined with indomethacin in preventing pancreatitis after endoscopic retrograde cholangiopancreatography(ERCP).Methods Patients who received ERCP were randomly divided into control group and treatment group.Control group was given somatostatin 6 mg intravenously for 24 h after ERCP.Treatment group was given indomethacin 0.1 g at 30 min before ERCP,given to the anus,on the basis of control group.Serum amylase(AMS),lipase(LPS),white blood cell(WBC),clinical efficacy,pain degree and the incidence of adverse drug reactions in two groups were observed.Results There were 50 cases in control group and 50 cases in treatment group.Seven days after treatment,the total effective rates of control group and treatment group were 80.00％(40 cases/50 cases)and 96.00％(48 cases/50 cases),with significant difference(P＜0.05).After 4 days of treatment,the serum amylase levels in treatment group and control group were(39.76±25.71)and(132.61±36.69)U·L-1;serum lipase levels were(52.96±14.87)and(108.67±14.87)U·L-1;WBC count were(5.31±1.45)× 109·L-1 and(9.31±3.45)×109·L-1,the differences were all statistically significant(all P＜0.05).The pain levels in treatment group at 4 h and 24 h after surgery were(3.03±0.75)and(1.96±0.66)points;which in control group were(4.81±0.74)and(2.03±0.61)points,the difference was statistically significant(all P＜0.05).There was no significant difference in the incidence of adverse drug reactions between the two groups(P＞0.05).Conclusion Somatostatin combined with indomethacin suppository can effectively reduce the levels of serum amylase,lipase and white blood cell count in patients with pancreatitis after ERCP,and reduce the degree of pain.关键词
