糖尿病并发心肌病的危险因素分析及与NLRP3炎性小体表达水平的相关性研究
Risk factors of diabetes complicated with cardiomyopathy and thier correlation with expression of NLRP3 inflammatory bodies

Abstract
[Objective]To analyze the risk factors of diabetes complicated with cardiomyopathy and the correlation with the expression of NLRP3 inflammatory bodies.[Methods]In this study,120 patients with diabetes who were admitted from January 2021 to October 2022 were taken as the research object,and a retrospective study was carried out.Fifty patients with cardiomyopathy were included in the combined group,and 70 patients without cardiomyopathy were included in the unconsolidated group.The general data and clinical data of the two groups were collected and compared.The risk factors that could lead to diabetes with cardiomyopathy were summarized through statistical single factor analysis and logistic multiple factor regression analysis.The cardiac function indicators and the expression level of NLRP3 inflammasome between two groups of patients were compared,and the correlation between the two was tested using Spearman correlation coefficient.[Results]Statistical univariate analysis showed that there were some differences between the two groups in age,BMI,course of diabetes,blood sugar level,blood lipid level,insulin function,mitochondrial function,and autophagy.Logistic regression analysis showed that the duration of diabetes ≥10 years,fasting blood glucose(FBG)≥10 mmol/L,HbA1c ≥10％,verylow density lipoprotein(VLDL)≥1.0 mmol/L,regular insulin(RI)≥25 pmol/L,C-P ≥1.3 nmol/L,reactive oxygen species(ROS)expression rate ≥90％,ATP ≥20.33 U,high expression of LC3B,and high expression of p63 were all risk factors for diabetes complicated with cardiomyopathy.After testing,the left ventricular ejection fractions(LVEF)of the combined group was 45.35％±8.47％,lower than that of the non-combined group 52.33％±10.62％,left ventricular end diastolic dimension(LVDD)was 60.35±10.27 mm,and NLRP3 expression level was 8.45±2.26 ng/mL,both higher than that of the non-combined group(53.39±10.16 mm,6.16±2.44 ng/mL)(P＜0.05).According to Spearman correlation coefficient test,the expression level of NLRP3 is negatively correlated with LVEF and positively correlated with LVDD.[Conclusion]When the course of diabetes is more than 10 years,blood glucose and lipid levels are abnormally elevated,accompanied by insulin resistance,mitochondrial function damage and autophagy,diabetes patients have a higher risk of cardiomyopathy.The expression of NLRP3 will increase with the decrease of LVEF and the increase of LVDD,closely related to the occurrence of cardiomyopathy.关键词
