Shin Ⅱ期、Ⅲ期外耳道胆脂瘤的个体化手术方式探讨

Abstract
[Objective]To explore the surgical methods and efficacy of Shin stage Ⅱ and Ⅲ external auditory canal cholesteatoma(EACC).[Methods]Clinical data of 55 patients(56 ears)with stage Shin Ⅱ and Ⅲ external auditory canal cholestatoma admitted to the Wuxi Second People's Hospital from August 2018 to September 2022 were retrospectively analyzed.[Results]According to the scope of cholestatoma invasion,EACC resection was performed in all 36 ears in stage Ⅱ,including 12 ears with external auditory meatoplasty and 3 ears with external auditory meatoplasty plus tympanoplasty.In stage Ⅲ,EACC resection+tympanoplasty+external auditory meatoplasty were performed in 18 ears,and radical mastoidectomy+tympanoplasty+mastoid constriction were performed in 2 ears.Dry ear 3 to 10 weeks postoperatively.All patients were followed up for 5 months to 2 years,and no recurrence of cholesteatoma was found.[Conclusion]Endoscopic auricular surgery is the first choice for stage ⅡEACC,with quick recovery and little trauma.The surgical method of stage Ⅲ EACC should be individualized according to preoperative temporal bone high-resolution CT.For stage Ⅲ EACC with extensive cholesteatoma and mastoid invasion,radical mastoid resection under microscope+tympanoplasty+mastoid constriction should be used.For stage Ⅲ EACC with limited mastoid air chamber involvement,endoscopic external auditory canal cholesteatoma resection+tympanoplasty+external auditory meatoplasty can achieve good therapeutic efficacy.关键词
