Correlation between perimenopausal subclinical hypothyroidism and bone mineral density and bone metabolism indexes

Abstract
[Objective]To study the correlation between perimenopausal subclinical hypothyroidism(SCH)and glucose and lipid metabolism and bone mineral density(BMD),to clarify the harm of SCH to the body.[Methods]A total of 78 pcrimenopausal patients with SCH from January 2020 to December 2022 in Xingtai General Hospital of North China Medical and Health Group were selected as the research group,and 80 healthy women of the same age were selected as the control group.Fasting venous blood was collected to detect thyroid function-related indicators,blood glucose index,blood lipid index and BMD of left hip,greater trochanter,femoral neck and lumbar spine L2-4,the correlation between SCH and glucose and lipid metabolism and bone mineral density was analyzed.[Results]Serum thyroid stimulating hormone(TSH)in the study group was 5.12±0.63 mU/L,higher than that in the control group(3.28±0.48 mU/L),and the difference was statistically significant(P<0.05).There was no significant difference in the levels of free triiodothyronine(FT3)and free thyroxin(FT4)between the two groups(P>0.05).The levels of total cholesterol(TC)and low density lipoprotein cholesterol(LDL-C)in the study group were higher than those in the control group under the steady-state mode,and the difference was statistically significant(P<0.05).There were no significant differences in fasting blood glucose(FPG),2hPG,triglyceride(TG)and high density lipoprotein cholesterol(HDL-C)levels between the two groups(P>0.05).The L2-4 BMD and average BMD of the left hip,femoral greater trochanter,femoral neck and lumbar spine in the study group were lower than those of the control group,with statistical significance(P<0.05).TSH was positively correlated with TC and LDL-C(r=0.719,0.580,P=0.003,0.015).TSH was negatively correlated with BMD(r=-0.341,P=0.036).FT3 and FT4 were not correlated with TC,LDL-C and BMD(r=0.000,P=1.000).[Conclusion]There is a significant correlation between TSH and glucose metabolism,lipid metabolism disorders and BMD decline in perimenopausal SCH patients,suggesting that SCH is not only easy to cause glucose and lipid metabolism disorders,but also easy to cause BMD decline,increase diabetes,hyperlipidemia and bone mass risk of loosening.关键词
