老年2型糖尿病患者骨骼肌质量减少与轻度认知功能障碍相关性研究
Study on correlation between skeletal muscle mass reduction and mild cognitive impairment in elderly patients with type 2 diabetes mellitus

Abstract
Objective To investigate the correlation between body composition and mild cognitive impairment(MCI)in elderly patients with type 2 diabetes mellitus(T2DM).Methods From August 2019 to June 2023,elderly T2DM patients hospitalized in the Department of Endocrinology,Drum Tower Hospital affiliated to Nanjing University Medical School were retrospectively analyzed.The assessments of global cognition and cognitive domains function,including immediate memory,visuospatial ability,language,attention,delayed memory,working memory,processing speed and executive function were conducted for patients enrolled.Meanwhile,the patients'general clinical data,laboratory results and body composition analysis were collected.According to the diagnostic criteria of MCI,the patients were divided into normal cognitive function group and MCI group.Relative weight analysis was used to evaluate association between different body composition indicators and the risk of MCI.Multivariate logistic regression was used to identify risk factors for MCI in elderly T2DM patients.Results After matching for gender and education level,a total of 409 elderly T2DM patients were enrolled,including 187 patients with normal cognitive function and 222 patients with MCI.There were significant differences in total body water(TBW),fat free mass(FFM),skeletal muscle mass(SMM),basal metabolic rate(BMR),bone mineral content(BMC),percent body fat(PBF),fat mass index(FMI),and visceral fat area(VFA)between the two groups(all P<0.05).Relative weight analysis revealed that SMM had the most significant association with MCI(percent weight:14.81%).All patients were divided into tertiles according to SMM levels from low to high,with 138,137,and 134 cases,respectively.The incidence of MCI in the low and middle SMM tertile groups was significantly higher than that in the high SMM tertile group.The scores of processing speed,working memory and visuospatial ability of the low SMM tertile were worse than those of the high SMM tertile(all P<0.05).Multivariate logistic regression analysis showed that after adjusting for covariates,the low SMM tertile(OR=4.209,95%CI:1.709-10.366)and middle SMM tertile(OR=3.386,95%CI:1.839-6.235)were associated with higher risk of MCI in elderly T2DM patients.Conclusions Reduced skeletal muscle mass is associated with an increased risk of MCI in elderly type 2 diabetes patients.关键词
