老年2型糖尿病住院患者合并肌少症相关因素研究
Study on relative factors to sarcopenia in elderly inpatients with type 2 diabetes mellitus

Abstract
Objective To explore the influencing factors of elderly inpatients with type 2 diabetes complicated with sarcopenia.Methods A retrospective case-control study was conducted to analyze the data of elderly patients with T2DM hospitalized in the Endocrinology Department of Beijing Hospital between January 2020 and May 2023.Sarcopenia is diagnosed according to the Asian Working Group for Sarcopenia(AWGS)2019 diagnostic criteria and multivariate Logistic regression were used to analyze the influencing factors of sarcopenia in patients with type 2 diabetes.Results A total of 469 elderly patients with type 2 diabetes were hospitalized.The incidence of sarcopenia was higher in males than that of in females(65.3%vs.34.7%,P<0.05).The T2DM+Sar group was older than the T2DM group(P<0.01).49 patients were involved in T2DM+Sar group and 420 were involved in T2DM group.After propensity score matching(PSM),153 patients with type 2 diabetes without sarcopenia(T2DM group)and 45 patients with type 2 diabetes and sarcopenia(T2DM+Sar group)were included.After correcting confounding factors of gender and age,the BMI of the T2DM+Sar group was significantly lower than that of the T2DM group(P<0.01).The proportion of patients with a BMI<24kg/m2 was higher in the T2DM+Sar group than that of in the T2DM group(53.6%vs.11.1%,P<0.01).The proportion of patients with cerebrovascular disease and diabetic peripheral neuropathy was higher in the T2DM+ Sar group than in the T2DM group(P<0.05).Compared with the T2DM group,the T2DM+Sar group had a higher Morse fall score,lower ADL score,longer time to complete 5 sit-to-stands,lower hand grip strength,and thinner upper arm circumference.The TLM,BMC,and VAT were lower in the T2DM+Sar group than in the T2DM group(P<0.05).Logistic regression analysis showed that DPN(OR= 3.348,95%CI:1.252-8.955),VAT(OR=1.042,95%CI:1.005-1.079),and SUA(OR = 1.007,95%CI:1.002-1.012)were risk factors for sarcopenia in elderly patients with type 2 diabetes,while BMI(OR=0.512,95%CI:0.363-0.721)and BMC(OR=0.037,95%CI:0.008-0.186)were protective factors for sarcopenia in elderly patients with type 2 diabetes.Among elderly patients with a BMI<24kg/m2,SUA(OR=1.010,95%CI:1.003-1.017)was a risk factor for sarcopenia in patients with type 2 diabetes,while lower limb average leg circumference(OR = 0.624,95%CI:0.447-0.870),HDL-C(OR=0.195,95%CI:0.040-0.945),and TLM(OR=0.889,95%CI:0.797-0.990)were protective factors for sarcopenia in this population group of elderly patients with type 2 diabetes.Conclusions In the management of elderly patients with type 2 diabetes,it is not recommended to control BMI at a low level.Body composition may be more meaningful than BMI in predicting sarcopenia in elderly patients with type 2 diabetes.关键词
