Abstract
Objective To investigate the clinical manifestations,diagnosis,and treatment of Candida albicans abscess at the surgical site after retroperitoneal laparoscopic nephrectomy.Methods A retrospective analysis was conducted on the diagnosis and treatment of a patient with severe hydronephrosis and nonfunctional kidney who underwent retroperitoneal laparoscopic nephrectomy in the Department of Urology at the Affiliated Hospital of Zu-nyi Medical University in October 2020 and developed a Candida albicans abscess at the surgical site.Relevant literature was reviewed.Results The patient was diagnosed through urine and drainage fluid bacterial culture and imaging examination.After conservative treatment,there was no significant improvement.Therefore,an attempt was made to place a drainage tube through the sinus tract,but the drainage effect was not satisfactory.Later,precise puncture drainage of the abscess was performed under CT guidance,but the abscess was not completely eliminated.After the final treatment of abscess removal and drainage,as well as sinus resection,the patient im-proved and was discharged.Conclusion Inpatients with high-risk invasive fungal infections who require retroper-itoneal laparoscopic nephrectomy,if the preoperative urine culture is positive for fungi,empirical antifungal treatment should be used to reduce urine leakage during surgery,rinse thoroughly,and drain thoroughly to avoid the use of foreign objects such as biosynthetic materials.If there is a concurrent abscess at the surgical site after surgery,invasive fungal disease can be diagnosed when the puncture fluid or pus culture is positive for fungi.When antifungal treatment is not effective,early abscess and foreign body removal surgery may be the best treat-ment option.关键词
