Study on the Influencing Factors of Severity and Rehabilitation Time of Congenital Muscular Torticollis

目的:探讨先天性肌性斜颈(CMT)严重程度及康复时间影响因素,预测CMT的预后.方法:回顾性分析2017年7月至2020年6月在长沙市妇幼保健院儿童保健中心儿童康复科确诊并收治的CMT儿童73例,收集患儿及母孕期病史资料,记录两侧胸锁乳突肌(SCM)肌肉厚度、患侧与健侧SCM厚度之比(A/N)、患侧与健侧SCM厚度之差(A-N)、康复时间等临床资料.根据患儿性别、斜颈偏侧、分娩方式、胎位进行分组,比较不同组别的SCM厚度、A/N、A-N及康复时间,分析康复时间的相关因素.结果:(1)右侧CMT组较左侧CMT组相比,SCM更厚,A/N、A-N值越大,差异有统计学意义(P<0.05);两组康复时间比较差异无统计学意义(P>0.05).(2)男女两组之间SCM厚度、A/N、A-N、康复时间相比差异无统计学意义(P>0.05).(3)异常胎位组较正常胎位组比较,SCM更厚,A/N、A-N值越大,康复时间更长,差异有统计学意义(P<0.05).(4)阴道难产(简称难产)与阴道顺产(简称顺产)、剖宫产分娩相比,难产A/N、A-N值越大,康复时间越长,差异有统计学意义(P<0.05),SCM厚度差异无统计学意义(P>0.05).(5)SCM厚度、A/N、A-N与康复时间呈线性正相关(P<0.001),相关系数分别为0.425、0.497、0.489.诊断年龄、出生体重与康复时间不相关,差异无统计学意义(P>0.05).结论:右侧CMT患儿、异常胎位、难产CMT患儿SCM更厚,异常胎位及难产CMT更加严重,需要更长的康复时间.出生体重、诊断年龄、性别与CMT康复时间无关.

Objective:To investigate the factors influencing the recovery time of patients with congenital myotonic torticollis(CMT)and to predict the severity and prognosis of CMT.Methods:Retrospectively,73 children with CMT and admitted to the pediatric rehabilitation department of the Children's Health Center of Changsha Maternal and Child Health Hospital Affiliated to Hunan Normal University from July 2017 to June 2020 were analyzed.The clinical data of the children and maternal pregnancy history were collected,and the clinical data of the sternocleidomastoid muscle(SCM)muscle thickness on both sides,the ratio of the affected side to the healthy side SCM thickness(A/N),the difference between the affected side and the healthy side SCM thickness(A-N),and the rehabilitation time were recorded.The children were grouped according to their sex,oblique neck deviation,delivery method and fetal position,and the SCM thickness,A/N,A-N and recovery time of different groups were compared,and the rel-evant factors affecting the recovery time were analyzed.Results:(1):The SCM was thicker and the A/N and A-N values were greater in the right CMT group compared with the left CMT group,and the difference was statistically significant;the difference in recovery time between the two groups was not statistically significant(P>0.05).(2)There was no statistically significant difference in SCM thickness,A/N,A-N,and recovery time compared between the two groups of men and women(P>0.05).(3)The abnormal fetal position group had thicker SCM,larger A/N and A-N values and longer recovery time compared with the normal fetal position group,and the difference was statistically significant(P<0.05).(4)Compared with vaginal obstructed labor(referred to as obstructed labor)and vaginal normal labor(referred to as normal labor)and cesarean delivery,the greater the values of A/N and A-N of obstructed labor,the longer the recovery time,and the difference was statistically significant(P<0.05),and the difference of SCM thickness was not statistically significant(P>0.05).(5)SCM thickness,A/N,and A-N were linearly and positively correlated with recovery time(P<0.001),with correlation coefficients of 0.425,0.497,and 0.489,respectively.age at diagnosis,birth weight,and recovery time were not corre-lated,and the differences were not statistically significant(P>0.05).Conclusion:SCM was thicker in children with right-sided CMT,abnormal fetal position,and difficult CMT,and abnormal fetal position and difficult CMT were more severe and required longer recovery time.Birth weight,age at diagnosis,and gender were not related to the recovery time of CMT.
