Hemoperfusion in the extracorporeal circulation reduces the risk of postoper-ative acute kidney injury in the aortic dissection operation

Abstract
Objective To explore the effect of hemoperfusion during extracorporeal circulation in acute aortic dissection operation on reducing the incidence of postoperative acute kidney injury(AKI).Methods Patients diagnosed with acute type A aortic dissection and treated with Sun's procedure from July 2022 to October 2022 at our hospital were included.Baseline charac-teristics,procedure data,clinical and biochemical data and prognostic outcomes of all the patients were retrospectively collected for analysis.Results A total of 57 patients were enrolled in the study,23 of which received hemoperfusion during extracorpore-al circulation.The incidence of AKI was significantly lower in the hemoperfusion group than in the non-hemoperfusion group(43.5%vs.85.3%,P=0.001),especially the grade 1 and 2 AKI(34.8%vs.61.8%,P=0.046).The maximum of creatinine[121(82,218)vs.174(112,220)umol/L,P=0.216]and cystatin C[1.63(1.34,2.85)vs.2.58(1.68,4.23)mg/L,P<0.05]at 7 days after the surgery were lower in the hemoperfusion group than those in the non-hemoperfusion group.There was also a decrease in the maximum value of procalcitonin at 3 days postoperatively in the hemoperfusion group compared with the non-hemoperfusion group[10.72(7.28,27.00)vs.6.50(2.60,11.0)ng/mL,P=0.011].The concentration of cold-inducible RNA-binding protein in se-rum did not show significant differences in the hemoperfusion group compared with the non-hemoperfusion group[987(664,1335)vs.993(741,1451)pg/mL,P=0.733].Conclusions Extracorporeal circulation combined with hemoperfusion in acute aor-tic dissection operation can reduce the risk of postoperative AKI,especially for grade 1 and 2 AKI.关键词
