摘要

Abstract

Objective To investigate the correlation between enlarged perivascular space(EPVS)in basal ganglia region and extracranial carotid atherosclerosis in mild stroke.Methods A total of 120 patients with mild ischemic stroke admitted to the Department of Neurology,Wuhu Hospital of Traditional Chinese Medicine from January 2019 to March 2023 were divided into the carotid arteriosclerosis group(n=63)and the non-carotid arteriosclerosis group(n=57)according to the results of carotid color ultrasound.The basal ganglion EPVS were graded according to the brain MRI.The patients were divided into mild EPVS group(EPVS level 0 to 1,n=61)and moderate to severe EPVS group(EPVS level 2 to 4,n=59).General datas,laboratory indexs and basal ganglia EPVS grade were compared between the carotid arteriosclerosis group and the non-carotid arteriosclerosis group.Spearman correlation analysis was performed to analyze the correlation between carotid intima-media thickness(IMT),Crouse plaque score and basal ganglia EPVS grade.Univariate analysis and multivariate Logistic regression analysis were performed to analyze the risk factors affecting the severity of basal ganglia EPVS.Results The grade of basal ganglia EPVS in the carotid atherosclerosis group was higher than that in the non-carotid atherosclerosis group(P<0.05).Carotid IMT and Crouse plaque score were positively correlated with EPVS grade in basal ganglia(r=0.575,P<0.001;r=0.481,P<0.001).Univariate analysis and multivariate Logistic regression analysis showed that carotid IMT and Crouse plaque score were independent risk factors for moderate and severe EPVS in basal ganglia region in patients with mild stroke(P=0.046,0.009).Conclusion EPVS in basal ganglia is associated with extracranial carotid atherosclerosis in patients with mild stroke.It suggesting that the two may have the same pathophysiological mechanism.
