老年带状疱疹患者的临床特征及发生后遗神经痛的风险因素分析

Abstract
Objective To explore the clinical characteristics of elderly patients with herpes zoster(HZ)and analyze the risk factors for postherpetic neuralgia in elderly HZ patients.Methods The case data of 82 elderly HZ patients admitted to De-partment of Dermatology of People's Hospital of Huaibei City from March 2020 to February 2023 were retrospectively ana-lyzed.The clinical characteristics of elderly HZ patients were analyzed by descriptive analysis.Univariate analysis and binary Logistic regression analysis were performed to identify the risk factors for postherpetic neuralgia in HZ patients.Results De-scriptive analysis of clinical characteristics of elderly HZ patients showed that the main season of onset was autumn in 31 cases(37.80% ),the main site of onset was the thoracic and dorsal nerve area in 33 cases(40.24% ),the main triggering factor was viral infection in 30 cases(36.59% ),and the main clinical manifestations classification was gangrene type in 33 cases(40.24% ).34 HZ patients(41.46% )were accompanied by prodromal pain.25 ones(30.49% )experienced postnatal neu-ralgia.Univariate analysis shows that the proportions of patients≥75 years old,with gangrenous HZ,accompanied by prodro-mal pain and with severe pain in the acute phase in the postherpetic neuralgia group were higher than those in the non-posther-petic neuralgia group(P＜0.05).Binary logistic regression analysis showed that age≥75 years old(OR =3.401),clinical manifestations of gangrene type(OR =2.601),acute severe pain(OR =4.319)were the risk factors for the occurrence of posthumous neuralgia in elderly HZ patients(P＜0.05).Conclusion The onset of elderly HZ patients may have obvious sea-sonality.Their HZ type is commonly gangrenous,and the onset site is mostly in the thoracic and dorsal nerve area.The inci-dence of posthumous neuralgia in elderly HZ patients is high.Patients aged≥75 years,with gangrenous HZ and severe pain in the acute phase may be more likely to develop postnatal neuralgia.关键词
