体外膜肺氧合辅助治疗患者死亡危险因素分析
Analysis of death risk factors in patients with extracorporeal membrane oxygenation-assisted treatment

Abstract
Objective To analyze the risk factors patients undergoing extracorporeal membrane oxygenation(ECMO)auxiliary therapy within 7 days after weaning from the machine.Methods The clinical data of 112 patients with ECMO therapy were retrospectively analyzed and divided into death group(n=77)and survival group(n=35)based on survival within 7 days after weaning.The age,gender,body mass index,and logistic regression was used to analyze factors affecting the risk of death within 7 days after ECMO patients weaned from the past medical history.Then,vital signs and labo-ratory indicators,total days of hospitalization,ECMO mode and running time,APACHE Ⅱ score and other indicators were compared between these two groups through plotting ROC curve.Results APACHE Ⅱ score,admission with arrhythmia,arterial lactate(Lac),potassium ion(K+),pro-thrombin time(PT),operating time,ventilator application time before boarding,extracorporeal car-diopulmonary resuscitation(ECPR)There were significant differences in the clinical data between the death group and the survival group(P< 0.05).Binary logistic regression analysis showed that A-PACHE Ⅱ score,admission with arrhythmia,Lac and K+had a positive promoting effect on the risk of death.The ROC curve was drawn based on the results obtained by binary logistic regression,sug-gesting that the AUC value was 0.857(95%CI:0.788~0.926),with a sensitivity of 63.6%and a specificity of 97.1%.Conclusions APACHE Ⅱ score,admission with arrhythmia,Lac,K+,PT,ECMO running time,ventilator application time before boarding,and ECPR have certain value in pre-dicting death within 7 days after weaning of ECMO therapy patients.For patients with ECMO-assis-ted circulation who are admitted to hospital with arrhythmia,high APACHE Ⅱ score,high Lac,and high K+,the mortality rate within 7 days after weaning from the machine tends to increase.关键词
