目的 探讨糖皮质激素口服给药和经鼻腔雾化吸入用于慢性鼻窦炎患者功能性鼻窦内镜手术后的效果.方法 选取2019年12月至2022年12月于苏州平江医院确诊的慢性鼻窦炎患者60例作为研究对象,根据就诊先后顺序将患者进行编号(1~60 号),1~30 号口服醋酸泼尼松片(口服组),31~60 号采用丙酸氟替卡松雾化吸入用混悬液经鼻腔雾化吸入(雾化组),比较两组的治疗效果.结果 雾化组治愈率略高于口服组,但组间比较差异无统计学意义(P>0.05).治疗2 周后,两组鼻内镜Lnud-Kenndry评分降低,且雾化组低于口服组,差异有统计学意义(P<0.05).治疗2周后,口服组血清糖皮质激素水平明显降低(P<0.05),而雾化组血清糖皮质激素水平与治疗前比较差异无统计学意义(P>0.05),雾化组血清糖皮质激素水平明显高于口服组(P<0.05).雾化组不良反应发生率低于口服组,差异有统计学意义(P<0.05).结论 糖皮质激素经鼻腔雾化吸入应用于慢性鼻窦炎行功能性鼻窦内镜手术患者,更有利于术后鼻窦黏膜恢复,且安全性更高.

Objective To explore the effects of oral administration of glucocorticoids and nasal nebulization after functional endoscopic sinus surgery in patients with chronic sinusitis.Methods Sixty patients with chronic sinusitis diagnosed in Suzhou Pingjiang Hospital from December 2019 to December 2022 were selected as the study objects.According to the order of treatment,the patients were numbered(1~60)and orally administered prednisone acetate tablets(No.1 to 30).No.31 to 60,Fluticasone propionate was inhaled by aerosol suspension through the nasal cavity(atomization group).The therapeutic effect of the two groups was compared.Results The cure rate of atomization group was slightly higher than that of oral group,but there was no significant difference between groups(P>0.05).After 2 weeks of treatment,the Lnud-Kenndry score of nasal endoscopy in 2 groups was decreased,and the nebulizer group was lower than the oral group,the difference was statistically significant(P<0.05).After 2 weeks of treatment,the serum glucocorticoid level in the oral group was significantly decreased(P<0.05),while the serum glucocorticoid level in the atomized group was significantly higher than that in the oral group(P<0.05),while the serum glucocorticoid level in the atomized group was significantly higher than that before treatment(P<0.05).The incidence of adverse reactions in atomization group was lower than that in oral group,and the difference was statistically significant(P<0.05).Conclusion The application of glucocorticoids through nasal atomization in patients undergoing functional endoscopic sinus surgery for chronic sinusitis is more conducive to postoperative sinus mucosal recovery and has higher safety.
