Abstract

Objective:To explore the expression levels of serum microRNA-124-3p(miR-124-3p)and early growth response 1(EGR1)in patients with acute ischemic stroke(AIS)complicated with dysphagia and their predictive value. Methods:A retrospective analysis was conducted on 181 AIS patients hospitalized at the Department of Neurology of the Fifth Affiliated Hospital of Zhengzhou University from May 2020 to March 2023.Based on the results of swallowing function tests,patients were divided into a dysphagia group(62 cases)and a non-dysphagia group(119 cases).The expression levels of serum EGR1 were detected using enzyme-linked immunosorbent assay,while real-time RT-PCR was used to detect the expression levels of miR-124-3p in serum.The correlation between serum miR-124-3p,EGR1 expression levels,and baseline data of AIS patients was analyzed using Spearman and Pearson correlation analysis.The predictive value of serum miR-124-3p and EGR1 expression levels for concurrent dysphagia in AIS patients was analyzed using receiver operating characteristic(ROC)curve analysis.The influencing factors of dysphagia in AIS patients were analyzed using logistic regression analysis. Results:The expression levels of serum EGR1,age,and National Institutes of Health Stroke Scale(NIHSS)score in the dysphagia group were higher than those in the non-dysphagia group(all P<0.05).The expression levels of serum miR-124-3p,Barthel index,and proportion of patients with muscle strength grade>3 were lower in the dysphagia group than those in the non-dysphagia group(all P<0.05).In AIS patients with concurrent dysphagia,the expression levels of serum miR-124-3p were negatively correlated with age and NIHSS score(r=-0.514,-0.520,both P<0.05),and positively correlated with Barthel index and muscle strength grade(r=0.509,0.492,both P<0.05).The expression levels of EGR1 were positively correlated with age and NIHSS score(r=0.498,0.516,both P<0.05),and negatively correlated with Barthel index and muscle strength grade(r=-0.527,-0.494,both P<0.05).The area under the curve(AUC)for predicting concurrent dysphagia in AIS patients based on serum miR-124-3p,EGR1 expression levels alone,and in combination were 0.795,0.811,and 0.880,respectively.EGR1 was identified as an independent risk factor for dysphagia in AIS patients,while miR-124-3p was a protective factor(both P<0.05). Conclusion:AIS patients with concurrent dysphagia exhibit low serum miR-124-3p expression and high EGR1 expression.Clinically,the combined detection of both markers can be used for early assessment of dysphagia to improve prognosis.
