Abstract
Objective To explore the risk factors of hypoglycemia in premature infants and provide reference for the monitoring and management of neonatal blood glucose.Methods The clinical data of 536 preterm infants with a gestational age of less than 37 weeks were reviewed.The number of cases in which hypoglycaemia(blood glucose less than 2.2 mol/L)occurred was recorded,and the differences in clinical data between the hypoglycaemic and normal groups were compared.Risk factors for hypoglycaemia in preterm infants were analysed using a logistic regression model.Results Hypoglycaemia was observed in 82 out of 536 preterm infants,resulting in an incidence rate of 15.3%.The gestational age and birth weight of preterm infants in the hypoglycaemia group were lower than those in the normal group.Additionally,the proportion of small-for-gestational-age(SGA)infants,caesarean section,birth temperature below 36.5℃,hypothyroidism during pregnancy,gestational diabetes mellitus,and gestational hypertension were higher in the hypoglycaemia group than in the normal group.These differences were statistically significant(P<0.05).The logistic regression analysis showed that preterm infants are at risk of hypoglycaemia due to being small for gestational age(OR= 1.401),gestational diabetes mellitus(OR=2.746),gestational hypertension(OR = 1.525),and birth hypothermia(OR = 2.110),while vaginal delivery(OR= 0.596)is a protective factor against it.All factors were found to be statistically significant(P<0.05).Conclusions Preterm infants are at an increased risk of hypoglycaemia due to SGA,birth hypothermia,gestational hypertension,and gestational diabetes mellitus.Additionally,vaginal delivery has been shown to reduce the risk of hypoglycaemia in preterm infants.关键词
