Effect of Soft-channel Minimally Invasive Puncture and Drainage on Pa-tients with Cerebral Hemorrhage and Its Impact on Hematoma Clearance and Traumatic Stress Index

Abstract
Objective To investigate the effect of soft-channel minimally invasive puncture and drainage in patients with cerebral hemorrhage and its impact on hematoma clearance and traumatic stress index.Methods A total of 108 patients with cerebral hemorrhage from June 2019 to June 2022 were selected as the research subjects,and were divided into hard channel group(n =54)and soft channel group(n =54)by computer random method.The hard channel group underwent hard-channel minimally invasive puncture and drainage,and the soft channel group underwent soft-channel minimally invasive puncture and drainage.Surgery related indexes,traumatic stress indexes,neurological function indexes,neurotransmitters,cranial CT indexes,complications,and National Institutes of Health Stroke Scale(NIHSS)and Glasgow Prognosis Scale(GOS)scores at 3 months after surgery were compared between the two groups.Results The duration of operation,time to extubation and length of hospitalization days in the soft channel group were shorter than those in the hard channel group,and the removal rate of hematoma≥80%on the day and at 3 days after surgery was higher than that in the hard channel group(P<0.05,P<0.01).At 3 and 7 days after surgery,the levels of corticotropin releasing hormone,cortisol,corticotropin,neuron-specific enolase,neuropeptide Y,glutamic acid and aspartic acid in soft channel group were lower than those in hard channel group,while the levels of brain-derived neurotrophic factor and glycine were higher than those in hard channel group(P<0.05).At 3 and7 days after operation,the volume of midline displacement and perihematoma edema in the soft channel group was lower than that in the hard channel group(P<0.05).At 3 months after surgery,NIHSS score in the soft channel group was lower than that in the hard channel group(P<0.05),and there was no significant difference in GOS score distri-bution between the two groups(P>0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion Soft-channel minimally invasive puncture and drainage in the treatment of cerebral hemorrhage can improve the removal effect of hematoma,reduce stress response,regulate the level of neurotransmitters,and promote the early recovery of nerve function,with high safety.关键词
