复发性中耳胆脂瘤的修正手术方案及疗效观察
Clinical Outcomes of rRevision Ossicles Reconstruction Surgery and Revision Mas-toidectomy for Recurrent Middle Ear Cholesteatoma Patient:a Retrospective Study

Abstract
Objective To evaluate and analyze the clinical efficacy of staged reconstruction of the ossicular chain in patients with recurrent middle ear cholesteatoma who require revision surgery.Method A retrospective study was based on 127 patients with recurrent middle ear cholesteatoma who underwent revision surgery at the First People's Hos-pital of Yinchuan City from January 2014 to January 2023.All patients were divided into three groups:radical revision mastoidectomy group(RM)with 46 cases,1 staged ossicular chain reconstruction revision surgery group(ls-ORS)with 52 cases,and 2 staged ossicular chain reconstruction revision surgery group(2s-ORS)with 29 cases.Compare the mid-term and long-term postoperative efficacy of ABG and EOS-16 quality of life scales,as well as the incidence of postop-erative complications.Result The total dry ear rate after revision surgery for recurrent middle ear cholesteatoma was 90.55％,and the total incidence of recurrent cholesteatoma was 1.58％.There was no statistical difference(P＞0.05)be-tween the 1s-ORS group and the 2s-ORS group in the ABG group at 6,12,and 24 months after surgery.At 36 months af-ter surgery,the ABG of the 1s-ORS group was(30.51±10.51)dBHL,while the ABG of the 2s-ORS group was(25.84 ±9.62)dBHL.There was a statistical difference between the groups(P＜0.001).The postoperative EOS-16 score in the 1s-ORS group was lower than that in the RM group,with no statistically significant difference(P＞0.05).The postopera-tive EOS-16 score in the 2s-ORS group was lower than that in the RM group,with a statistically significant difference(P＜0.05).There was no statistically significant difference in the postoperative EOS-16 score between the 1s-ORS group and the 2s-ORS group(P＞0.05).Conclusion For patients with recurrent or wet ears after middle ear cholesteatoma sur-gery,revision surgery can significantly improve their quality of life.Patients who undergo 2 staged ossicular chain re-construction surgery may have better long-term auditory performance.关键词
