Respiratory interventional therapy for critical patients under extracorporeal membrane oxygenation
部分重症患者的呼吸介入治疗在常规局麻或全麻下风险极高,体外膜肺氧合(ECMO)可以提供有效的呼吸和(或)循环支持,预防或克服围手术期致命的低氧血症和高碳酸血症,保障手术顺利进行.文章结合目前发表的短期ECMO支持下重症患者呼吸介入治疗的病例报道,针对ECMO支持下呼吸介入治疗的应用时机、适用人群和ECMO管理细节(如工作模式、抗凝治疗、撤机和并发症等)等方面进行概述和探讨,以期能为更好开展ECMO支持下呼吸介入治疗提供指导.
Pulmonary interventional therapy for some critical patients poses significant risks under routine local or general anesthesia.Extracorporeal membrane oxygenation(ECMO)can provide effective respiratory and/or circulatory support and prevent or overcome perioperative life-threatening hypoxemia and hypercapnia,ensuring the success of the operation.Combined with the currently published case reports on respiratory interventional therapy in critical patients with short-term ECMO,this paper summarizes and discusses the proper time of application,suitable population and details of ECMO management(such as working mode,anticoagulant therapy,weaning and complications)of respiratory interventional therapy with the support of ECMO,with the aim of providing guidance for better carrying out respiratory interventional therapy with the support of ECMO.
