基于慢性疾病轨迹框架的糖尿病患者的自我管理教育干预及效果分析
Analysis of the Efect of Self Management Education Based on the Framework of Chronic Disease Trajectory for Diabetes Patients

Abstract
Objective:To explore the effect of self-management education intervention for diabetes patients based on chronic disease trajectory.Methods:400 outpatient and inpatient patients from a hospital in Dongguan City from January 2022 to May 2023 were selected,including 200 patients in the initial stage and 200 patients in the stable stage.According to the intervention method,they were divided into observation groups and control groups,with 100 cases in each group.The observation groups received targeted self-management education and support based on the patient's disease stage,while the control groups received unified and undifferentiated self-management in-formation push notifications.Fasting blood glucose were measured before intervention,at the end of the first,third and sixth month of the intervention.Glycated hemoglobin were measured before intervention,at the end of the third and sixth month of intervention.Self-management level were measured before intervention and at the end of the sixth month of intervention.Results:There was no statistically significant difference in fasting blood glucose,glycated hemoglobin concentration,and self-management level between the two groups of patients in the initial and stable stages before intervention.At the end of the first month,the third month and the sixth month,the fast-ing blood glucose of the observation groups were significantly lower than the control groups in both two stages(P<0.05).At the end of the third and sixth month of intervention,the glycated hemoglobin of the observation groups were significantly lower than the control groups in both two stages(P<0.05).At the end of the sixth month of in-tervention,the self-management scores of patients in observation group of the initial stage were significantly high-er than that in the control group(P<0.05).Conclusion:The self-management education intervention effect of di-abetes patients based on the chronic disease trajectory framework is better than that of conventional self-manage-ment health education,with more targeted and personalized support,so as to improve patients'self-management knowledge,promote the formation of self-management behavior,and stabilize patients'blood sugar and glycosy-lated hemoglobin.关键词
