激素性股骨头缺血性坏死是长期或过量使用糖皮质激素引起的一种非创伤性疾病.现代研究发现,自噬作为一种细胞内调节机制,其与激素性股骨头缺血性坏死之间存在一定的相关性.而PI3K/Akt/mTOR信号通路是干预细胞自噬的关键环节,对细胞代谢平衡具有重要的病理生理作用.明确自噬在该通路的调控下对激素性股骨头缺血性坏死致病过程的影响,将有助于从分子水平认识该病,并期望通过靶向调节自噬水平,达到延缓激素性股骨头缺血性坏死进程的目的.因此,本文就PI3K/Akt/mTOR信号通路、细胞自噬及激素性股骨头缺血性坏死三者之间的联系进行综述,以期为临床靶向治疗激素性股骨头缺血性坏死提供新的视角及理论依据.

Steroid-induced avascular necrosis of the femoral head is a non-traumatic disease caused by long-term or excessive use of glucocorticoids.Modern studies have found that autophagy,as an intracellular regulatory mechanism,has a certain correlation with steroid-induced avascular necrosis of the femoral head.The PI3K/Akt/mTOR signaling pathway is a key link in the intervention of autophagy,plays an important pathophysiological role in the balance of cell metabolism.To clarify the effect of autophagy on the pathogenesis of steroid-induced avascular necrosis of the femoral head under the regulation of this pathway will help us to understand the disease at the molecular level,and hope to delay the process of steroid-induced avascular necrosis of the femoral head by targeting the regulation of autophagy.Therefore,this article reviews the relationship between PI3K/Akt/mTOR signaling pathway,autophagy and steroid-induced avascular necrosis of femoral head,in order to provide a new perspective and theoreti-cal basis for clinical targeted therapy of steroid-induced avascular necrosis of femoral head.
