目的:探讨基于灰阶超声构建症状性颈动脉斑块影像组学模型的可行性及应用价值.方法:由两名超声医师独立收集颈动脉斑块病例分别作为训练集和验证集.在训练集和验证集内部根据患者近期是否出现对应的脑缺血症状分为症状组和非症状组.比较两组患者一般临床资料情况.提取基于灰阶超声图像的颈动脉斑块影像组学特征.在训练集中,以是否存在脑缺血症状为因变量,以斑块影像组学特征为 自变量,采用LASSO回归筛选自变量构建影像组学模型.采用ROC曲线、校准曲线及决策曲线对模型进行内部验证和外部验证.结果:训练集和验证集内部症状组和非症状组一般临床资料比较差异无统计学意义(P均>0.05).训练集中共精选出6个斑块影像组学特征构建模型,该模型在训练集中的AUC为0.913[95%CI(0.862~0.964)],C-index为0.887;在验证集中的 AUC 为 0.837[95%CI(0.758~0.917)],C-index 为 0.798.训练集和验证集中校准曲线与理想曲线拟合良好,DCA曲线净获益值高.结论:基于灰阶超声构建影像组学模型识别症状性颈动脉斑块具有可行性及一定的应用价值.

Objective:To explore the feasibility and application value of constructing symptomatic carotid plaque radiomics model based on gray-scale ultrasound.Methods:Carotid plaque cases were independently collected by two sonographers as a training set and a validation set.In the training set and validation set,patients were divided into symptomatic group and asymptomatic group according to whether they had corresponding cerebral ischemia symptoms recently.The general clinical data of the two groups were compared.The radiomics features of carotid plaque based on gray scale ultrasound images were extracted.In the training set,with the presence or absence of cerebral ischemia as the dependent variable and the plaque imaging features as the independent variable,LASSO regression was used to screen the independent variables to construct the radiomics model.The ROC curve,calibration curve and decision curve were used to verify the model internally and externally.Results:There was no significant difference in the general clinical data between the cerebral infarction group and the non-cerebral infarction group in the training set and the verification set(P>0.05).In the training set,six radiomics features were selected to build the model.In the training set,the AUC and C-index of the model were 0.913[95%CI(0.862-0.964)]and 0.887.In the validation set,the AUC was 0.837[95%CI(0.758-0.917)]and the C-index was 0.798.In the training set and verification set,the calibration curve fits the ideal curve well,and the DCA curve has high net benefit value.Conclusion:The radiomics model based on gray-scale ultrasound is feasible and valuable to identify symptomatic carotid plaque.
